Nurse led primary care interventions: a literature review to identify acceptable effective models
This project was undertaken under a contract for the Department of Health and Ageing (DOHA). It involved a literature review of Australian and international literature examining nurse-led primary health care interventions, models of care and services with a particular focus on those serving chronic and complex conditions and hard to reach populations.
