‘Social referral initiatives’ are a transformative approach for linking Aboriginal and Torres Strait Islander people with chronic conditions to a range of various services and supports. Connecting people from primary care to services and supports will address the unmet holistic needs of individuals. This research will evaluate how people with chronic conditions can be best supported to access services and participate in activities to manage their conditions and improve their quality of life.
